Facility: Crestwood Solano PHF

Job Title: Licensed Clinician (LMFT/LCSW)

Status: Part Time

Shift: AM

Minimum Qualifications:
1. A minimum of 2 years in a healthcare setting at least one of which must have been in a mental health setting.

2. Demonstrated knowledge of DSM-5.

3. Familiarity with DMH Titles 9 and other governmental regulations.

4. Licensed as a LMFT or LCSW


Duties:
1. Functions as the interdisciplinary team coordinator for an assigned caseload of clients to ensure the timely implementation of the service plan.

2. Collaborates with the Interdisciplinary Team, which develops, monitors, and refines the service plan to achieve identified client outcomes.

3. Assist with the Daily Treatment Focus meetings in which the progress of the clients are reviewed and recommendations are made regarding the implementation of the overall service plan.

4. Demonstrates ability to facilitate the task group process to achieve clearly defined client goals, objectives and responsibilities.

5. Functions as the client’s advocate to facilitate effective communication and treatment.

6. Assures that the client is oriented to the program.

7. Assures that the client participates, as feasible, in the development and implementation of the service plan.

8. Within the program philosophy, goals and objectives, ensures the client is receiving individualized care to best meet identified needs and strengths.

9. Functions as a liaison to the family to facilitate effective communication.

10. Oversees and assists with provisions of clients services according to the individual service plan.

PHYSICAL DEMANDS:

1. Must be in good physical condition to meet demands of an active position, necessitating extensiveness of walking and standing.

2. Must be able to stoop, crouch, reach, and stand, moving about in work area and throughout facility to accomplish task.

3. Must be able to get in and out of vehicles and tolerate one to two hours of driving for transportation of clients.

4. Must be able to assist with the physical transfer and restraint of clients.

5. Must be able to lift a minimum of 25 pounds.
