Facility: Crestwood Crisis Stabilization Services

Job Title: Service Coordinator

Status: Full Time

Shift: AM

Minimum Qualifications:
This job will be located in Fairfield, CA

Education and Experience:
Prefer a bachelor’s degree in psychology, social work, or a field related to mental health.
Two (2) years of full-time experience or part-time equivalent in a mental health setting.
Demonstrated knowledge of DSM-V and psycho social rehabilitation and recovery principles.
Familiarity with regulatory standards.
Proven ability to work with others, at all levels within the organization, and collaborate effectively.
Above-average interpersonal, problem-solving, and written and oral communication skills.
Experience in working with health care and mental health regulatory agencies.
Meet minimum competencies or design plan to meet competencies within one year of employment.


Duties:
1. Program Responsibilities:

Functions as the interdisciplinary team coordinator for an assigned caseload of clients to ensure the timely implementation of the treatment plan.
Collaborates with the interdisciplinary team, which develops, monitors, and refines the treatment plan to achieve identified consumer outcomes.
Coordinates regular meetings in which the progress of the clients is reviewed and recommendations made regarding the implementation of the overall treatment plan.
Demonstrates ability to facilitate the task group process to achieve clearly defined client goals, objectives, and responsibilities.
Functions as the client’s advocate to facilitate effective communication and treatment.
Assures that the client is oriented to the program.
Assures that the client participates, as is feasible, in the development and implementation of the treatment plan.
Within the program philosophy, goals, and objectives, ensures the client is receiving individualized care to best meet identified needs.
Assists clients in community integration through outings, social activities and community entry.
Functions as a liaison to the family to facilitate effective communication.
Maintains contact with the family during facility stay.
Arranges family conferences as the team or family request it.
Facilitates and coordinates the discharge planning process.
Ensures discharge planning goals and objectives are developed and reviewed at each IDT conference.
Ensures activities related to discharge are implemented.
Provide clinical services that meet the needs outlined in the recovery treatment plan.
Completes the required 52 hours of yearly in-service training.
Works with clients in training capacity, if applicable.
All other duties as assigned.
2. Documentation Responsibilities:

For each assigned client, completes required documentation and assessment.
Completes assessment.
Documents interventions and contacts in the medical record.
Completes necessary entries in the medical record as specified by policy and within the necessary timeframe.
Recovery service plan
Weekly note
Monthly summary
Quarterly summary
Any daily notes pertaining to the client’s treatment.
3. Quality Improvement:

Ensures the quality and timeliness of medical record documentation in accordance with CBHI-defined policies and procedures.
Concurrently and retrospectively reviews medical charts to ensure that the goals are functional and realistic and that the criteria for medical necessity are being met.
Demonstrates knowledge of an awareness of the standards of the regulator’s licensing and accrediting agencies governing the facility.
4. Referral Development:

Interfaces with and acts as liaison to external service coordinators.
Functions as liaison to the referral sources, community, agency, etc., as indicated to facilitate communication regarding client care and programs that may be utilized by these sources. Markets the services of the facility to potential referral sources.
PHYSICAL DEMANDS:

Must be able to lift at least 25 lbs.
Cognitive skills to move from one task to another and to attend to detail.
Assist with physical interventions.
